Passport


If you’re making plans for international travel, you can apply for a U.S. Passport at the Camdenton and Osage Beach Library branches.

These library branches offer passport services for NEW applicants. To renew your passport, please fill out and mail in form DS-82, an agent is available to review for accuracy. The Library does not process passport renewal applications. Passport photos can be obtained from Walmart, Walgreens, or CVS.

The passport acceptance process must be completed by the stated end times below. The process takes approximately 30 minutes per each application, so please plan accordingly.

In order to apply for a passport book or card, please read Form DS-11 Application for A U.S. Passport.

Copies of this form are available at the Library or can be downloaded from the State Department website. If you want to fill out your application from before you come in, use black ink and leave it unsigned – do not sign the application until instructed to do so during the application process.

Applicants must complete the following checklist. Missing information will result in a delay in your passport processing time.

All person applying for a passport must be present in person, regardless of age.

  • Completed Application
    (COMPLETE LEGIBLY IN BLACK INK. Mistakes should be crossed out with a single line with the correction written above. No white-out or scribbling.)
    • Form DS-11: Application for a U.S. Passport
      (DO NOT SIGN the application or attach your photo until instructed to do so during the application process.)
  • Payment for Applicable Fees
    Two separate fees are payable at the time of application
    • Check (not starter/counter), cashier’s check or money order made payable to the U.S. Department of State. Families may write one check or money order to the U.S. Department of State for their passport application fees if they are applying in person at the same time. For all other applicants, separate checks must be made for each application. (See fees section.)

AND

  • Cash, money order, debit/credit card, or check made payable to Camden County Library District for execution fee of $35.
  • Proof of U.S. Citizenship
    (Bring one of the following original documents or a certified copy.)
    • Birth certificat issued by the city, county, or state
    • Previously issued, undamaged U.S. passport**
    • Consular Report of Birth Abroad or Certification of Birth
    • Naturalization Certificate** or Certificate of Citizenship**
      ** Also requires submission of certified birth certificate if applicant is age 15 or younger.
  • Most Recent Passport (if applicable)
  • Primary Identification
    (Bring one of the following:)
    • Valid Missouri driver’s license or Photo ID(If out-of-state, will need to additional ID. See https://travel.state.gov for more information on acceptable IDs.)
    • Previously issed, undamaged U.S. Passport, current foreign passport or current green card
    • Naturalization Certificate
    • Current Military ID (military or dependents) or government ID (city, state, or federal)
    • For children & teens: For applicants age 16 and 17, parent/legal guardian may persent primary ID if the minor applicant does not have one. For ages 15 and younger, see below.
  • Appearing Parent/Legal Guardian (For applicants age 15 and younger.)
    • Parents/guardians must present primary identification, as listed above.
    • Applicants age 15 and younger need both parents/legal guardian(s) present to apply
    • If only one parent/legal guardian can appear, form DS-3053 must be submitted with application.
    • If one parent/guardian has sole custody, certified proof of sole custody must be submitted.
    • Applicants age 16 & 17: It is recommended that at least one parent appear in person with the applicant to show parental awareness.
  • One Passport Photo
    • Applicant must have one official color photograph taken within the last six (6) months.
    • Photograph should be 2 inches x 2 inches, full-face, front view, no glasses, hair pulled back behind ears, with a plain white or off-white background on photo paper.

    • The Library does not offer passport photos, but Walmart, Walgreens, and CVS can take passport photos.

State Department Fees

Please refer to the U.S. Department of State’s passport fees for up-to-date information on how much it costs to obtain a passport book and/or card.

Adult (16 or older): First Time or Not Eligible for Renewal

When you apply for a passport using form DS-11, you will need to pay two fees: one to the U.S. Department of State and one to the facility where you submit your application.

Child (Under 16): First Time

When you apply for your child’s first passport using Form DS-11, you will need to pay two fees: one to the U.S. Department of State and one to the facility where you submit your application.

    Library Fees

    • The $35 processing fee per application is paid to the Camden County Library District, and can be paid with a money order or check made out to Camden County Library District, or you can use a credit/debit card*.
    • There is an additional charge of $31.42** for 1 – 2 day expedited mailing to the Philadelphia, PA facility.

    *If using a credit or debit card, there is a convenience fee of $2.00 or 2.9%, whichever is larger.
